Pebble Creek of Akron Honored with Quality Improvement Award Facility One of Few in State to Achieve Gold and Platinum Status

AKRON, Ohio— Pebble Creek skilled nursing facility is one of just 38 facilities in the state to earn “Gold Level” recognition for its participation in the Ohio Nursing Home Quality Care Collaborative (NHQCC) initiative managed by Health Services Advisory Group. This group is Ohio’s Medicare Quality Improvement Organization.

There are approximately 1,000 skilled nursing facilities in Ohio, so Pebble Creek is among an elite group—just 4 percent in the state–to earn Gold status. It is also one of just 28 facilities—or just 3 percent in Ohio–to earn Platinum status.

“We take very seriously our job to provide warm, expert care for every resident at Pebble Creek,’’ said Cari Templeton, the facility’s executive director. “We provide individualized care and focus, from Day 1 of admission, on getting residents well and back home as soon as possible.”

Ohio’s effort to improve care at nursing facilities through these Gold and Platinum status programs focuses on several areas, including:

Reducing healthcare-acquired conditions.
Reducing antipsychotic medication rates.
Reducing avoidable hospital readmissions.
Participating in Quality Assurance & Performance Improvement initiatives.
Increasing influenza and pneumococcal vaccination rates.
Reducing staff turnover and implementing consistent assignment.
Improving rates of resident mobility.
Earning a Quality Measure Composite Score of 6 percent or less, based on the 13 long-stay quality measures publicly reported on Nursing Home Compare.
Pebble Creek earned its “Gold Level” recognition by actively participating in NHQCC quality-improvement learning opportunities, designating staff members for its NHQCC team, assessing its current QAPI status, and identifying potential performance improvement project opportunities within the facility.
