The process of admission to any skilled nursing facility can be overwhelming. If you desire, one or more members of our friendly team of professionals will meet you at the hospital or your home to address any questions or concerns you may have about your care or that of your loved one. We will also provide you with a list of items to bring to the facility to ensure that you are prepared to take the next step with us.
Our admissions coordinator is available to answer questions while you fill out the required forms and discuss the payment options available to you. To aid in the process, we kindly ask that you bring copies of all of your insurance cards at the time of admission.
Plan of Care
During your stay in our skilled nursing facility, the interdisciplinary team will meet to discuss the individualized plan of care for you or your loved one. You and/or your family are always invited to attend these meetings, which address both physical and emotional needs. Progress reports can be provided to personal physicians to ensure continuity of care.
