Prior Admission
The process of admission to any skilled nursing facility and senior rehabilitation center can be overwhelming. If you desire, one or more members of our friendly team of professionals will meet you at the hospital or your home to address any concerns you may have about your care or that of your loved one. We will also provide you with a list of items to bring to the center. If you or a loved one is entering long term care, this list starts you off on the right foot while providing some of the feelings of home.
Admission
Our admissions coordinator will sit with you and/or your responsible party to fill out the required forms and discuss the payment options available to you. To aid in the process, we kindly ask that you bring copies of all of your insurance cards.
Plan of Care
During your stay, the interdisciplinary team will meet to discuss the individualized plan of care for you or your loved one in our skilled nursing home or long-term care center. You and/or your family are always invited to attend these meetings, which address both physical and emotional needs. Progress reports can be provided to personal physicians to ensure continuity of care.
Payment/Admission
There comes a time for many families when 24-hour skilled nursing care for a loved one is simply a necessity. When this time arrives, being surrounded by an experienced clinical team, who cares like family, is essential to one’s quality of life. At CommuniCare, we understand this and know that quality of life cannot be achieved by tending only to an individual’s clinical needs. Instead, we focus on delivering high quality of care to meet an individual’s complete needs in an engaging and vibrant environment.
