Welcome

Advanced Specialty Hospital of Toledo is a 40-bed, long-term acute care hospital in Toledo, Ohio. We specialize in caring for patients that are medically complex, stable and require extended hospitalizations. We welcome those from short-term acute care hospitals, intensive care, step down, or cardiac units when these hospitals are no longer appropriate or cost-effective. The Advance Specialty Hospitals of Toledo is proud to be accredited by the Joint Commission, registered by the state of Ohio. We are also Medicare-certified and offer our patients the highest standard of care.

Step-By-Step

  1. Prior Admission

The process of admission to any facility can be overwhelming. If you desire, one or more members of our friendly team of professionals will meet you at the hospital or your home to address any concerns you may have about your care or that of your loved one. We will also provide you with a list of items to bring to the facility.

  2. Admission
    • We accept patients with various medical conditions and physical disabilities. Our admission requirements for patients include the following:
      • Patient requires 8-12 hours of direct nursing care each day.
      • Patient is expected to require acute care for an extended period of time.
      • Patient is medically stable for transfer.
      • Patient’s health status is likely to improve.
      • Patient will benefit from the involvement of an interdisciplinary team.
      • Patient requires daily physician visits.
      • Patient requires hospital level care (i.e., on a ventilator or involved in an accident, surgery or sudden life-changing healthcare event).

Anyone can initiate a referral, however a physician order is required for each admission. Free pre-admission screening assessments are available to determine if your patient is a candidate for Advanced Specialty Hospital of Toledo.

  3. Plan of Care
  • Admissions accepted 24 hours a day, 7 days a week.

Our Approach

We pride ourselves on working together as a close-knit family to provide tailored care and support for each of our guests. Much of our success in improving patient outcomes is the result of a low nurse-to-patient ratio and the fact that our caregivers get to know our patients and family members over their extended hospital stays. By offering a smaller, personalized environment, we can focus our attention on providing patients the highest level of care as well as ready access to many of the resources of a full-service hospital. This seamless continuum of care is essential for optimum healing and health.

Our patients benefit from the unique, state-of-the-art LTACH environment, which provides acute medical care in a professional, university-quality environment. This maximizes patients’ recovery options. Long-term acute health care differs from nursing homes and subacute care in many ways. Our patients are medically complex requiring daily physician visits, critical care ACLS trained nurses, respiratory therapists, pharmacists, dietitians, social workers, discharge planners, dialysis technicians and physical and occupational therapists and speech language pathologists.

Lifestyles

Our specialized treatment programs include treating the whole person through a well-planned aggressive integrated program of medical and therapeutic care. Our programs include the following:

The Pulmonary/Ventilator Program: This provides patients with respiratory failure the opportunity to be successfully weaned from ventilator dependence and achieve their highest functioning level. Because of the continuity of care over an extended period of time, many of our patients can return home breathing on their own.

Medically Complex Program: Patients receive integrated treatment for more than one condition, including life-support services that can include telemetry, potential nutritional support, cardiac and respiratory monitoring, and dialysis.

Complex Wound Program: Patients with pressure ulcers, serious wound complications or who are recovering from extensive surgery receive specialized treatment through this program.

Low Tolerance Rehabilitation Program: This program offers physical and occupational therapy, and speech language pathology geared to patients who are unable to participate in more than three hours of therapy a day.
